Eligible people have until Wednesday, November 4, to file a claim in the Summit Medical Group data settlement. File online by that date or have a paper claim postmarked by then. The settlement administrator lists the date without a cutoff hour or time zone.
The settlement covers living U.S. residents whose private information may have been compromised in the September 2024 incident, including people Summit Medical mailed a notice. The online claim form asks for the LoginID and PIN from that notice. If you lost it or think you qualify without having received one, the administrator gives instructions for requesting those credentials.
What you can claim
The settlement FAQ says people with unreimbursed losses related to the incident can seek up to $2,500. Submit receipts or similar records to support those losses. A separate claim for time spent responding to the incident pays $15 an hour for up to three hours, with a maximum of $45. The form asks claimants to attest to their time. Eligible claimants can request both cash benefits and two years of medical data monitoring.
Cash amounts may be reduced if approved cash claims exceed the $500,000 cap for those payments. HIPAA Journal also reported on the settlement and the November 4 claim deadline.
The court has scheduled a final approval hearing for November 19 at 10 a.m. Central time. Benefits depend on final approval and the resolution of any appeals. The administrator gives no guaranteed payment date.
